TARGET GROUP ELIGIBILITY: People with a disability (intellectual, physical, sensory, neurological, cognitive) who are under the age of 60 at the time they apply for support.
DESCRIPTION OF SERVICE: Local Area Coordinators (LAC) work with individuals with disabilities and their families, assisting them to plan, select and receive the supports that they need. LACs also work with community groups and other agencies to help make communities more welcoming and responsive to the needs of people with disabilities. COSTING OF SERVICE: There is no cost.
FUNDING BODIES: State government funded.
GEOGRAPHICAL COVERAGE: The Lower Great Southern office covers from Walpole to the west, up to Cranbrook, across to Jerramungup and Ongerup. Local Area Coordinators have offices across the whole of WA, covering every town and district.

ELIGIBILITY: Person must have a diagnosed-mental illness and be referred by their Mental Health Officer or Doctor.
DESCRIPTION OF SERVICE: Fellowship House is open weekdays between 9:30am and 2:30pm as a "drop-in" centre. We provide transport to and from the house if required (in Albany Town Area). Tea/Coffee and lunches are provided for a small cost. We encourage participation in recreational activities such as golf, ten pin bowling, bushwalking, art class, sewing and computing, to name a few. _________________________________________________

ELIGIBILITY: People with a psychiatric disorder and are experiencing moderate to severe disability as a consequence of the disorder; people with a high risk of developing a psychiatric disorder; people who are a high risk of suicide; people at risk of developing a psychiatric disorder who would benefit from targeted early intervention that is not provided by primary services on the private system. _________________________________________________

Welcome to Activ, a not-for-profit organisation dedicated to promoting a better life by providing a range of choices and opportunities for people with disability and their families living and working in the community.
From finding a job to choosing a place to live, Activ creates an environment in which people with disability can achieve their best. We offer innovative employment support, alternatives to employment programs, all ages respite, retirement planning, exciting recreation and travel services as well as the day-to-day assurance of a quality transport service.

The type of work available includes: • Property Care – contract mowing and landscaping • Textiles – sewing for various contract jobs; • Industrial Services – packaging, folding newsletters; repotting plants, sorting and cutting rag for industry; • Building Products – park benches, arbours and lattice work.

Accommodation
Our permanent group homes and short-term housing options provide a quality and caring environment for residents. Living in a group home, with our skilled and dedicated staff, residents are encouraged to be involved in all aspects of domestic home-life, including, cooking, cleaning, gardening and entertaining. Group homes, each accommodating five or six residents, are located throughout the State. Activ adheres to the State Disability Standards for Accommodation and Community Services. Recreation Whether it's for two weeks or one day, there's nothing quite as good as taking a break to enjoy new and exciting experiences. At Activ Recreation we offer a huge range of holiday and recreation programs that give adults with intellectual disability the chance to discover the world, as well as their own backyard! Programs range from overseas holidays to half-day excursions organised by Activ staff members and supported by our enthusiastic volunteers. Activ's Alternatives to Employment Service offers a choice of community and centre based activities in an environment that enables participants to form friendships and learn new skills.
Alternatives to Employment programs are generally provided in community based settings. Activities are selected to ensure that they benefit the individual to assist in achieving their goals and outcomes.
Activ's Alternatives to Employment Service provide support to people with disability who are not able to maintain full time employment. Activ provides a range of services that encourage involvement in everyday community life while promoting skill development, enjoyment and satisfaction.
